Toolbox talk sign-off sheet
Shaun Holmes
I've spent over 20 years in UK construction, and a briefing that nobody signs is a briefing I cannot show later. This is a blank attendance register: the talk, the site, the date, who presented it, and a line for each person who was there.
This talk supplements the site's own risk assessment and method. It does not replace them, and it is not a substitute for competent health and safety advice.
How to use it
Write the talk title in the blank at the top. If the topic is already written up, open it from the library and use the sheet on that page, which has the title filled in. If you are picking the topic from the year, use the 52-week planner. If you are writing the points yourself, use the blank toolbox talk template, which has room for the key points as well as the names.
Fill in the site, the date, the supervisor, and the time you started. Read one topic. Leave a gap for a question. Then pass this sheet round. Name, trade, signature. You sign it at the end and write the time you finished. Keep it with the job records.
Signing means the person was at the briefing and had the chance to ask. It does not replace the risk assessment or the method, and it does not train anyone for work they are not competent to do. The page stays open to read. Printing asks for an email the first time.
Attendance and sign-off
Print this sheet for the people who were in the briefing. It is one A4 page on its own when you print the sheet. The whole talk prints with the sheet on a new page. Pop your email in the first time. After that, the buttons print straight away.
